Ages and Stages Questionnaires (ASQ) are a series of development questionnaires that assess children's development across different developmental domains such as physical, cognitive, language, social-emotional, and self-help skills. ASQ assessments allow parents and healthcare professionals to identify areas where a child is thriving as well as the areas in need of attention.

ASQ questionnaires are designed for children between 4 months and 5 years old. Parents or guardians should be the primary responders for the questionnaire; however, other caregivers such as teachers or daycare providers can also complete the questionnaire if they have regular contact with the child.

Completing an ASQ assessment provides insight into your child's development that may not be easily identified in extracurricular activities or daily interactions. Identifying any potential developmental delays early on ensures that you can get your child the help they need in order to reach their full potential.

After completing an ASQ assessment, you will receive a personalized report outlining your child's strengths and weaknesses across all five domains of development as well as tips for promoting further growth in any areas of concern. The report will include recommendations for next steps, such as seeking additional assistance from a professional health care provider or engaging in certain types of activities to facilitate growth in specific developmental domains.

It is recommended that children between 4 months and 5 years old take the ASQ assessment at least twice a year, at least 4 weeks apart from each other. This allows you to track changes over time while also checking up on any areas of concern more frequently than once a year.

The questionnaire consists of age-appropriate questions ranging from items about physical milestones (crawling or walking) to questions about social-emotional skills (sharing toys). Answers require simple Yes/No responses or rating scales based on frequency (such as “never”, “sometimes”, “often”).
