What does BHH mean in HEALTHCARE

Brooklyn Health Home is a program designed to provide holistic, comprehensive care for people with complex medical needs. It works in partnership with both healthcare and social-service providers to coordinate and deliver better care.

To be eligible for the Brooklyn Health Home, one must be a resident of Brooklyn with multiple chronic health needs, including physical disabilities, intellectual/developmental disabilities or mental illness.

The program operates with a team-based approach that connects people to all aspects of their health care. Through coordinated services, individuals receive comprehensive assessment, regular check-ups, preventive screening and medication management. In addition, referrals to other specialists and social activities are made available as needed.

Services provided through the Brooklyn Health Home include primary care coordination, disease management support, medication management and medication assistance programs, behavioral health services, home health services and other supportive services as needed.

There is no cost associated with participating in the program. All services are provided at no cost unless otherwise stated by your insurance provider.

Yes! We offer a variety of educational workshops and activities focused on staying healthy and active within our community such as nutrition classes, exercise classes and stress reduction techniques. In addition we offer additional resources such as smoking cessation programs and substance use disorder treatment referrals upon request

Absolutely! You can bring along a friend or family member if you would like additional support during your appointment.

Once enrolled in the program you will be given an initial appointment date within two weeks from enrollment date. Subsequent appointments will be scheduled according to individual needs.
