What does IRD mean in HEALTHCARE

Infantile Refsum disease (IRD) is a rare and potentially life-threatening genetic disorder that primarily affects the eyes, heart, skeletal structure, central nervous system, and skin. It is caused by a deficiency of an enzyme called phytanoyl-CoA hydroxylase. Symptoms may include poor vision, increasing deafness, weak muscle tone, impaired motor skills, and mental retardation.

IRD usually develops shortly after birth due to a lack of the enzyme phytanoyl-CoA hydroxylase which breaks down certain fatty acids into smaller molecules for energy in cells. Without this enzyme certain fatty acids will build up in the body leading to various symptoms associated with IRD.

The signs and symptoms of IRD vary depending on severity but can include poor vision; increasing deafness; weak muscle tone; impaired motor skills; mental retardation; seizures; joint stiffness; metabolic acidosis; short stature; scoliosis (curvature of the spine); enlarged liver or spleen; cardiac defects such as atrial septal defect (ASD) or ventricular septal defect (VSD);deafness or hearing loss due to nerve damage in the inner ear; intracranial calcifications (abnormal deposits of calcium within the brain); epiphyseal dysplasia (abnormal cartilage development at the end of long bones in the arms and legs);and respiratory complications due to accumulation of fluid in lung tissue.

Diagnosis of IRD typically involves a thorough physical exam including blood tests that measure levels of specific enzymes related to fatty acid metabolism. Imaging scans such as CT scans or MRI's may also be used to detect abnormalities such as intracranial calcifications caused by deficient fatty acid metabolism. Genetic testing may also be used to confirm diagnosis if ordered by a healthcare provider.

There is no known cure for IRD as it is caused by mutations to genes related to fatty acid metabolism that are present from birth. Treatments focus on managing symptoms and preventing complications from developing over time. Treatment options may involve medications, surgery for some cardiac conditions, physical therapy for impaired motor functions, hearing aids for deafness or hearing loss due to nerve damage in the inner ear and surgical interventions for scoliosis if necessary.
