What does HFOC mean in HEALTHCARE

HFOC partners with area organizations and businesses to fund programs that provide health education, preventive care services, financial assistance, community outreach initiatives, medical home support services and more. Additionally, HFOC works in collaboration with local provider networks such as hospitals and clinics to ensure high-quality care with a patient-centered focus.

Individuals that meet established criteria including financial need, age requirements and/or other criteria related to specific program eligibility may qualify for HFOC services or benefits. HFOC offers several innovative programs such as health coaching, financial assistance programs for prescriptions, home visits from social workers who diagnose unmet medical needs like vision or mobility aids; and parental training classes focused on healthy child development topics; among others. As new needs arise in our community we strive to develop solutions that expand access and improve the quality of life for all residents of Orange County. Yes! Mental health is just as important as physical health; therefore, we have partnered with various providers throughout Orange County who are committed to helping those struggling with mental illness find the help they need quickly and easily. Our goal is to ensure that individuals receive comprehensive care from qualified experts in the most timely manner possible.
