The most common symptoms of acalculous cholecystitis include right upper abdominal pain, nausea and vomiting, fever, jaundice and tenderness in the right upper abdomen. Other signs may include mild jaundice, generalized abdominal tenderness, abdominal distention and low-grade fever.

Acalculous cholecystitis can be challenging to diagnose due to its non-specific or atypical presentation. Diagnosis usually involves blood tests, imaging studies such as CT scans or ultrasound scans, and a percutaneous transhepatic cholangiogram (PTC) to evaluate for biliary obstruction or dilation of intrahepatic or extrahepatic bile ducts.

Treatment for acalculous cholecystitis typically includes conservative measures such as antibiotics and supportive care to manage pain and decrease inflammation. Surgical removal of the gall bladder (cholecystectomy) may also be recommended in some cases if medical management is not effective.

If left untreated or misdiagnosed with other conditions such as appendicitis or pancreatitis, AC can lead to serious complications such as sepsis (blood infection), gangrene (death of tissue) of the gallbladder wall or abscess formation around the gallbladder which can cause bile duct obstruction leading to jaundice.

Surgical treatment for AC carries a risk for complications including infection at the surgical site and damage to surrounding organs such as the liver or intestines during surgery.
