What does CNRT mean in COMMUNITY

CNRT stands for Community Neuro Rehabilitation Team. This is a specialist team of healthcare professionals who provide specialist assessments, treatments and interventions to adults with acquired brain injuries or neurological conditions as well as complex long-term conditions.

The CNRT provides services to adults aged 18+ who have been affected by an acquired brain injury or neurological condition, such as stroke, Multiple Sclerosis (MS) or Motor Neuron Disease (MND). It focuses on helping people manage their ongoing health care needs in the community and may also provide rehabilitation programmes where needed.

The CNRT can provide a wide range of support depending on individual need and patient's goals. This includes specialized assessment, treatment and intervention services such as physiotherapy, occupational therapy, speech therapy and cognitive behavioural therapy sessions; advice on lifestyle changes; home visits; case management plans; signposting to other relevant services; medication reviews and monitoring; and coordination of care planning meetings with all involved professionals.

To access support from the CNRT one must first be referred by a GP or other health professional. All referrals will be assessed based on clinical need, before any further action is taken. Once accepted onto the service individuals will be given an appointment at their local community neuro rehabilitation team base clinic to complete their comprehensive assessment and discuss treatment options.

Depending on local arrangements this service may be free depending on your eligibility for certain schemes such as Continuing Healthcare or Social Services funding packages. For those not eligible for these schemes there may be some cost associated with this service but your local NHS Trust should be able to advise you further about this if required.
