What does CCTP mean in HOSPITALS

The Community-Based Care Transitions Program (CCTP) is a Medicare grant program that was created to improve outcomes for high-risk Medicare beneficiaries who are discharged from the hospital and transitioning back into their community. CCTP helps patients make sure that they have access to necessary care and supports following their transition, so they can continue their recovery in the comfort of their own home.

Qualification for CCTP requires a referral from a health care provider, like a hospital or nursing home.
