What does GCCV mean in GERIATRIC

The purpose of the GCCV is to ensure that geriatric patients in Central Virginia receive timely, quality, and effective care. Some of the services offered through the GCCV include access to interprofessional clinical teams, development of evidence-based clinical pathways for common geriatric conditions such as falls prevention and chronic pain management, educational programs for both professionals and caregivers, telehealth interventions, medication reconciliation services, home visits, outreach programs for underserved populations, transition support for those transitioning from one setting or provider to another, as well as social work supports.
